Willow Review 2026: Women-Focused GLP-1 Prescribing With Oral Options

Willow is the GLP-1 sister brand to Winona (women's HRT). They offer compounded semaglutide from $299/month and tirzepatide from $399/month, with both injectable and oral sublingual tablet options. Flat pricing that does not change with dose. The women-focused positioning and connection to Winona's HRT services create a natural pathway for menopausal women dealing with both hormonal symptoms and weight gain. But a recent NAD referral to state attorneys general and polarized Trustpilot reviews warrant caution.

Pricing

MedicationCost
Compounded semaglutide (injectable or oral)$299/month (flat at all doses)
Compounded tirzepatide (injectable)$399-$524/month
Ondansetron (anti-nausea add-on)$30/month

All-inclusive: medication, consultation, physician messaging, and free 2-3 day shipping. No hidden fees.

At $299/month for semaglutide, Willow is more expensive than Hims ($199/mo), IVY RX ($175/mo), or Henry Meds ($197/mo). The premium is partly the oral tablet option and partly the women-focused clinical team.

How It Works

  1. Online eligibility assessment and health history
  2. Same-day physician approval (per marketing)
  3. Personalized prescription with 2-3 day delivery
  4. Ongoing doctor messaging and care team support

Willow will prescribe to patients outside traditional BMI requirements for “cosmetic” weight loss, which is unusual. Most platforms require BMI 27+ with a comorbidity or BMI 30+.

What I Like

Oral Sublingual Semaglutide Tablet

Willow offers a daily oral dissolving semaglutide tablet as an alternative to weekly injections. For women who are needle-averse, this removes the biggest barrier. This is a compounded formulation (not the FDA-approved oral Wegovy). In the STEP 1 trial, semaglutide 2.4mg produced 14.9% mean weight loss over 68 weeks, with 86.4% of participants losing at least 5% of body weight (NEJM, 2021). Willow also offers tirzepatide, which outperformed semaglutide in the SURMOUNT-5 trial (20.2% vs 13.7% weight loss).

Women-Focused Positioning

Willow’s clinical team and messaging are designed for women. Combined with their connection to Winona for HRT, a woman dealing with both menopause symptoms and weight gain can get hormone therapy and GLP-1 treatment through affiliated platforms with overlapping physicians.

Flat Dose-Agnostic Pricing

Like IVY RX, Willow charges the same regardless of your dose level. No $100 upcharge as you titrate up.

What Could Be Better

NAD Referral to State Attorneys General

The National Advertising Division (BBB National Programs) found that Willow could not substantiate its weight loss claims using brand-name semaglutide studies for compounded formulations. Willow refused to comply with modification recommendations. NAD referred the case to state attorneys general in December 2025. This is an active, unresolved regulatory issue.

Polarized Reviews

49% of Trustpilot reviewers gave 1 star. That is an unusually negative split. Common complaints include shipping delays, orders arriving without medication, and difficulty getting refunds.

Only 33-34 States

Willow does not serve 16-18 states. Most major competitors like Hims and Ro serve all 50.

Oral Tablets Have Mixed Results

Some users report zero weight loss on the oral sublingual tablets. The compounded oral format has not been clinically validated the way FDA-approved oral Wegovy has. Approach with appropriate expectations.
