Quick Answer (Updated April 2026)
Cash-pay pricing for online GLP-1 medications in 2026 ranges from $80 to $600 per month depending on medication and provider. Compounded semaglutide median is $199, compounded tirzepatide median is $279, and branded GLP-1 programs median is $119. The cheapest options come from established telehealth platforms with national footprints, while the most expensive are typically in-person clinics or premium concierge services.
Key Findings
- Cheapest compounded semaglutide program is Maximus at $80 per month. Most expensive is JumpstartMD at $349.
- Cheapest compounded tirzepatide program is Maximus at $132 per month. Most expensive is MyDietDoc at $600.
- Across all compounded tirzepatide programs, half charge $279 per month or less. The other half charges more, sometimes much more, for similar medication.
- Branded GLP-1 programs (Wegovy, Zepbound, Ozempic, Mounjaro through telehealth or in-person clinics) range $74 to $450 with a median of $119. List price for these medications is usually $900 to $1,400 per month, so providers in this index typically reflect manufacturer savings programs, vial-form pricing, or clinic visit fees rather than full list price.
- Programs that include metabolic labs, clinician support, or free shipping skew toward the higher end of each category. Programs with medication only (no clinician check-ins) cluster at the lower end.

Methodology
This index tracks cash-pay monthly pricing for online and in-person GLP-1 weight loss programs in the United States. Prices are pulled from each provider's public-facing website and verified against the program checkout flow where available. We re-verify pricing on the first of every month and stamp the page with the most recent check date.
What we track
- Monthly headline price. The fee a new patient sees on the provider's main pricing page for a starter program. We use the introductory or month-one price when one exists, and note this in the program field.
- Medications offered. We list compounded semaglutide, compounded tirzepatide, and branded medications (Wegovy, Zepbound, Ozempic, Mounjaro) separately because pricing dynamics differ across categories.
- What's included. Whether the price covers medication only, or also includes clinician support, metabolic labs, free shipping, or app access.
- Geographic coverage. Number of US states served, including telehealth-only providers and in-person clinic networks.
What we exclude
- Programs without published cash-pay pricing.
- Programs priced below $50 per month, which usually reflect non-medication services (diet companion subscriptions, coaching-only memberships) rather than GLP-1 medication access.
- Insurance-billed pricing. List prices for branded medications run $900 to $1,400 per month before insurance. The "Branded GLP-1 Programs" table reflects what providers charge cash-pay patients, which often involves manufacturer savings, vial-form pricing, or visit fees rather than full list price.
- Promotional first-month-only discounts when the long-term steady-state price is materially higher.
Limitations
Pricing changes frequently in this market. Providers occasionally run promotions, raise prices, or restructure programs without changing their public pricing page. Where we find a discrepancy between a provider's stated price and what a patient sees at checkout, we flag the row and re-verify. This dataset reflects a single point in time and should be treated as a directional reference, not a substitute for checking the provider's current pricing before signup.

Frequently Asked Questions
How much does compounded semaglutide cost online in 2026?
Across the 68 compounded semaglutide programs we track, monthly prices range from $80 to $349. The median is $199 per month. Most established telehealth providers price compounded semaglutide between $129 and $249 per month, with cheaper options usually requiring an annual commitment and more expensive options including labs and clinician check-ins.
How much does compounded tirzepatide cost online in 2026?
Across the 52 compounded tirzepatide programs in this index, monthly prices range from $132 to $600. The median is $279 per month. Tirzepatide is consistently more expensive than semaglutide due to higher API cost. The cheapest tirzepatide programs cluster around $179 to $249 per month.
What is the cheapest GLP-1 program online?
The cheapest compounded semaglutide program in this index is Maximus at $80 per month. The cheapest compounded tirzepatide program is Maximus at $132 per month. Note that price alone does not capture program quality, clinician access, medication sourcing, or shipping reliability. See our cheapest GLP-1 programs ranking for tradeoffs.
Why is compounded tirzepatide cheaper than Zepbound?
Compounded GLP-1s are produced by licensed compounding pharmacies using the active ingredient (semaglutide or tirzepatide) rather than the FDA-approved branded product. Compounding is permitted under sections 503A and 503B of the Federal Food, Drug, and Cosmetic Act, and pricing is not constrained by the manufacturer list price of $900 to $1,400 per month for Zepbound or Wegovy. The cash-pay price difference reflects compounding pharmacy economics, not insurance subsidies.
How is this pricing data verified?
We pull each provider's published monthly price from their main pricing page, then cross-check against the new-patient checkout flow when available. Prices are re-verified on the first of every month. The page footer shows the most recent verification date. If a provider's price has changed since our last check, the row is updated and noted in the monthly change log.
